A patient with paroxysmal nocturnal hemoglobinuria compurpura fulminans carried a pregnancy to successful delivery. All cases of pregnancy in patients with paroxysmal nocturnal hemoglobinuria reported in the English language are described and discussed. Suggestions for the management of pregnancies in such patients are presented, including prophylactic transfusions and puerperal anticoagulation. © 1982 by The American College of Obstetricians and Gynecologists.
