Inflammatory pseudotumour of the lung (IPT), also termed as plasma cell granuloma is non-neoplastic process due to unregulated growth of inflammatory cells, usually sequelae to old lung infection. These tumours are exceptionally rare, that are usually present as a solitary nodule. Here, we present a case of 25-year-old male complaining of dry cough, dyspnoea and intermittent fever since three years with multiple large, lobulated intraparenchymal mass lesions in mid and lower zones of both lung fields. Transthoracic CT guided biopsy was reported as inflammatory pseudotumour. The imaging features, clinical presentation and differential diagnosis of this seldom encountered entity are discussed which can present also as multiple intraparenchymal masses. © 2008 Elsevier Ireland Ltd.
