In an attempt to identify a focus of infection, WBC scintigraphy was performed, and it showed increased accumulation in the midanterior pelvis suspicious for a site of infection. Computed tomography of the pelvis obtained after WBC scintigraphy showed gross retention of stool and a distended bladder, but no suggestion of infection. Repeat WBC scintigraphy performed with a Foley catheter in place after bowel cleansing did not show any abnormal tracer accumulation. This case shows how the combination of a distended bladder and fecal retention can produce images suggestive of a site of infection.
